Developmental Word Study Consent Form

I have read and understand the information about collecting survey information to examine participants' knowledge and use of developmental word study. Some of this information may be used in conference presentations and professional papers. Any information used will not include my actual name.  I understand that I can receive information about the results if I request it. I also understand that analysis of the data will occur after course grades are submitted. My consent is voluntary, and I am aware that I can stop my participation at any time.
